Your First Visit
When you call for an appointment, our staff will gather basic information from you and schedule an appointment that will be reserved exclusively for you. We will mail you a welcome packet containing information about our practice, as well as a few confidential forms to be completed and brought with you to your first visit. You may also print these forms from this sit for your convenience. Please assist us at the time of your initial visit to the office by providing the following information: your referral slip and
x-rays from your referring dentist; a list of medications you are currently taking or may be allergic to; and your driver's license and dental insurance cards. Please notify the office if you have a medical condition or concern prior to surgery (e.g., artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
By bringing these completed Forms to your first appointment, the doctor will be able to see you promptly for your consultation.
At your initial consultation appointment, one of our specialists will complete a thorough periodontal examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease, as well as a full evaluation of your medical and dental history. Your doctor will then explain his findings, make an accurate diagnosis and discuss possible treatment options; thus ensuring you have a comprehensive understanding of the nature and severity of your condition.
After you and the doctor have chosen the best option, our treatment coordinator will meet with you to make financial arrangements so that you may find the best fit for your current budget and schedule the appointment for your procedure. Occasionally surgery can be performed on the same day as the consultation. However, a complex case and at the patient's discretion, the surgery may be scheduled at a later date.
For your convenience, our well-trained staff are proficient in maximizing your dental benefits and doing the utmost needed for insurance compliance. At your request, we will submit your treatment plan to your dental insurance company prior to the procedure for a "predetermination". The predetermination provides us with an estimate of your dental coverage for the specific procedures submitted. This estimate is not a guarantee by your insurance company; however, it gives you the best approximation of your fees for the treatment you need. Please remember that your dental benefits are a contract between your employer and the insurance carrier.
Furthermore, we offer our patients interest-free financing and options that allow you to spread payments over time yet complete treatment promptly. For your convenience, we accept cash, check, Visa, MasterCard and Discover. Interest-free financing for up to 12-months is available through Care Credit.
